The six categories with concrete examples in each, what they look like in an office, a shop floor, and a kitchen, the two groups small employers almost always miss, and the control ladder that decides whether a fix survives a busy week

The first hazard list I wrote for my own company had eleven items on it, and every one of them could cut, burn, or fall on somebody. It felt thorough. It was also the wrong list.

What cost us actual days that year was a stack of boxes narrowing a fire route, a laptop set on a shelf at chin height for a month, and one person taking every closing shift because nobody had looked at the schedule as a safety question. None of the three would have made my list, because my instinct only searched for things with edges.

That is what the hazard categories are for. Sorting a workplace into physical, safety, chemical, biological, ergonomic, and psychosocial forces you to look in the places instinct skips. This guide gives concrete examples in each category, what they look like in an office, a shop floor, and a kitchen, and the order of controls that decides whether a fix is still working in three months. What Counts as a Workplace Hazard

A workplace hazard is any condition, substance, activity, or way of organizing work that has the potential to cause injury or illness to a person at work. Potential is the operative word: nothing has to have happened yet for something to qualify.

A source of potential harm to a worker, whether that harm arrives in one second or over ten years. Hazards include physical agents such as noise and heat, chemical and biological substances, mechanical and electrical energy, the physical demands a task places on the body, and the way work is designed, scheduled, and supervised. A hazard exists independently of whether anyone is currently exposed to it, which is why hazard identification is a separate exercise from investigating what has already gone wrong.

Hazard and risk are not synonyms, and mixing them up is the most common reason an assessment turns into a list of adjectives. The hazard is the drum of solvent. The risk is how likely someone is to be harmed by it and how badly, which depends on how often it is opened, by whom, and with what ventilation.

The scale is worth holding in mind, because it explains why the categories most people skip are the expensive ones. Private industry employers reported 2.5 million recordable nonfatal injuries and illnesses in 2024, a rate of 2.3 cases per 100 full-time equivalent workers and the lowest count in a data series going back to 2003, according to the Bureau of Labor Statistics Survey of Occupational Injuries and Illnesses released in January 2026. Almost none of those cases involved a hazard nobody had ever seen before.

Legally, the backstop is broad. Beyond the specific standards that apply to your industry, the general duty clause requires a workplace free from recognized hazards likely to cause death or serious physical harm, which is why a hazard with no standard attached is still your problem.

The Six Hazard Categories

Workplace hazards are conventionally sorted into six categories: physical, safety, chemical, biological, ergonomic, and psychosocial. Some frameworks list five by folding safety hazards into the physical group, which is the same content organized differently rather than a disagreement about what is dangerous.

Damage is usually gradual, and by the time it shows up in a hearing test or a heat collapse the exposure has been running for months.

These are the hazards most people picture, they produce the injuries that arrive in a single second, and they dominate the federal fatality counts. Many taxonomies fold this group into physical hazards; keeping it separate is more useful on a walkthrough.
ChemicalSubstances that harm on contact or inhalationCleaning products, solvents, fuels, adhesives, paints, dusts, welding fume, and pesticides. Every workplace has some, because degreasers and bleach are chemicals regardless of the industry printed on the door. Hazard communication is consistently one of the most cited federal standards, and offices are not exempt from it.

PsychosocialHow the work is organized and ledLow job control, effort and reward that do not match, job insecurity, long hours, sustained overload, and violence or abuse from customers and coworkers. The federal occupational health agency treats these as workplace hazards rather than as personal problems, and points employers at changing the work before coaching the worker.
Six categories, and the last two are where small employers find hazards they did not know they had. A list that stops at the first two is the list most businesses actually write.

Keeping safety separate from physical is a practical choice rather than an academic one. The two groups behave differently on a walkthrough: safety hazards announce themselves in a single moment, while physical hazards accumulate quietly and need a measurement or a question rather than a glance.

The categories are also a useful audit of your own attention. If your existing list is entirely safety hazards, you have written down what you are afraid of rather than what is harming people, and the fix takes an afternoon.

Physical Hazard Examples

Physical hazards are forms of energy in the environment that act on the body without any substance being ingested or any contact being made: noise, heat, cold, vibration, radiation, poor lighting, and air pressure. The harm is usually cumulative, which is exactly why they get missed.

Noise deserves a line of its own because of how quietly it works. The National Institute for Occupational Safety and Health (NIOSH) puts about 27 million workers exposed to hazardous noise each year, and treats noise as hazardous at 85 dBA and above. It also counts roughly 22 million workers exposed to ototoxic chemicals, which damage hearing on their own and worsen the effect of noise, a combination almost nobody checks for.

The Arm’s Length Test
You do not need a meter to start. If two people standing an arm's length apart have to raise their voices to hold a conversation, the noise level in that space is worth measuring properly. It is the same test safety consultants use to decide where to bother with a dosimeter, it costs nothing, and it will tell you in one walk whether noise belongs on your hazard list at all.

Safety Hazard Examples

Safety hazards are the conditions that cause sudden traumatic injury: walking surfaces, work at height, unguarded machinery, electricity, vehicles, confined spaces, and blocked egress. They are the most familiar group and still the deadliest.

Federal fatality data makes the ranking concrete. There were 5,070 fatal work injuries in 2024, a rate of 3.3 per 100,000 full-time equivalent workers. Transportation incidents were the most frequent fatal event at 1,937 cases, or 38.2 percent of the total, and fatal falls, slips, and trips accounted for 844 (Census of Fatal Occupational Injuries, 2024).

Two patterns run through that table. The first is that most of these appear during non-routine work: cleaning, unjamming, restocking, and repairing. The second is that they cluster where space is temporarily borrowed. Chemical Hazard Examples

Chemical hazards are substances that harm through contact, inhalation, ingestion, or injection, and every workplace has them, because bleach, degreaser, and toner are chemicals regardless of the industry on the door. Harm ranges from a same-day burn to an illness that appears decades later.

The compliance side is unusually well defined here. Hazard communication is consistently one of the two most frequently cited federal standards, and it reaches any workplace that stores chemicals rather than only the ones that manufacture them. The obligations are a written program, safety data sheets employees can actually reach, labels on every container including the ones you filled yourself, and training that covers what is in the building.

The Unlabeled Spray Bottle Is the Whole Standard in Miniature
A cleaner decanted into an unmarked bottle fails the labeling requirement, defeats the safety data sheet, removes the information an emergency responder would need, and makes a dangerous mixture more likely. The fix is a label and a marker. If you audit one chemical thing this quarter, audit the containers your own people filled, because the purchased ones are almost always labeled correctly and the refilled ones almost never are.

Biological Hazard Examples

Biological hazards are living organisms and the material they leave behind: bloodborne pathogens, respiratory viruses, mold, contaminated food and water, and pests. Healthcare is the obvious case, and it is a long way from the only one.

The federal bloodborne pathogens standard is the one small employers most often assume does not apply to them. It applies where occupational exposure to blood or other potentially infectious material is reasonably anticipated, and it asks for a written exposure control plan, engineering and work practice controls, protective equipment, and training. A designated first aid responder in a non-medical business can bring you inside it.

Illness data moves faster than injury data, which is a reason to revisit this category more often than the rest of the inventory. Federal survey figures put private industry illness cases at 148,000 in 2024, down 26.0 percent, driven by a 46.1 percent fall in respiratory illness cases to 54,000, the lowest count in that series since 2019. A category that can move by nearly half in a single year is not one to copy forward from an older inventory without looking.

Ergonomic Hazard Examples

Ergonomic hazards come from the fit between the job and the body: lifting, repetitive motion, awkward or static posture, sustained force, and equipment set up for an average person who does not exist. This category produces the largest single group of serious injuries in federal data.

Across private industry in 2023 and 2024, overexertion, repetitive motion, and bodily conditions accounted for 946,290 cases involving days away from work, restricted duty, or transfer, the highest of any event group, ahead of contact with objects and equipment at 860,050. Cases involving days away from work ran to 1.8 million over the two years, with a median of 8 days away per case (Bureau of Labor Statistics).

The tell for this category is behavioral rather than physical. People stop asking for help with a lift, start bracing a wrist, or quietly swap a task with a colleague. None of that reaches an incident report, so the only reliable route to finding it is asking, and a structured ergonomic assessment checklist gives the question a shape.

What worked for me
The most useful ten minutes I have ever spent on ergonomics involved no equipment at all. I asked three people to show me the part of their job they would change if they could, and then watched them do it. One was carrying a monitor box up a half flight of stairs because the elevator was slow, one was reaching across a desk for a scanner forty times a morning, and one had been working with a screen far below eye height since the day she started. Two of the three were solved by moving something. The third cost the price of a stand. Nobody had ever reported any of it, because none of it felt like an injury while it was happening.

Psychosocial Hazard Examples

Psychosocial hazards are factors in the work environment that can cause stress, strain, or interpersonal problems for the worker, and they are treated as workplace hazards rather than as personal shortcomings. NIOSH names low job control, an imbalance between effort and reward, job insecurity, long working hours, job strain, and workplace violence among the examples (NIOSH science bulletin on work-related psychosocial hazards).

Two of those rows are usually treated as management problems rather than safety problems, which is the mistake. In 2018 survey data cited in the same bulletin, nearly 30 percent of workers frequently found their work stressful and around a quarter reported that they lacked decision-making power or could not take needed time off. The recommended response starts at the organizational level, because changing working conditions is described as more effective, broader in impact, and more sustainable than individually focused support alone.

Workplace violence has its own well-established typology worth knowing when you assess who is exposed. Type I is criminal intent by someone with no relationship to the business, Type II is a customer, client, or patient attacking a worker, Type III is worker on worker, and Type IV involves someone from an employee's personal life. Fatal violence was recorded in 733 cases in 2024, of which 470 were homicides. Which type your business faces determines the control, and it is the question workplace violence prevention training should start from.

One clarification, because the vocabulary collides. Psychosocial hazards are conditions of work that can cause harm. Psychological safety is a team climate where people can speak up without fear of humiliation. They are related and not interchangeable: a team with high psychological safety will report psychosocial hazards faster, which is useful precisely because the hazards still exist.

The Same Six Categories in Three Real Workplaces

The categories are constant and the contents are not. Here is what each one holds in three settings a small business actually occupies, which is the fastest way to see that no workplace scores zero anywhere.

Read down your own column and the point lands. An office has entries in all six rows, and the two that people assume belong to industry, chemical and biological, are occupied by a closet of cleaning products and a first aid kit. The difference between settings is severity, not category coverage.

This is also the table that decides what your health and safety policy has to name. A policy that lists hazards you do not have and omits the ones you do is worse than a short one, because it signals to anyone reading it that nobody walked the building.

How to Find the Hazards in Your Own Workplace

Hazard identification is a walk plus a set of questions, done while the work is happening rather than before opening. The categories give the walk a structure so it does not become a search for the things you already worry about.

Walk it at working speed
Mid-shift, with the machines running and the deliveries arriving. A workplace inspected at 7 a.m. on a quiet morning is a different workplace from the one your team occupies at 2 p.m. on a Friday, and only one of the two has hazards in it.

Take one category at a time
Six passes beat one comprehensive sweep, because each category needs different senses. Noise and heat need you to stop and notice. Ergonomic hazards need you to watch a full cycle of a task. Psychosocial hazards need you to look at a schedule rather than a room.

Ask the people doing the job
The single most productive question is what nearly went wrong recently, because near misses live in people’s heads rather than in your records. The second is what part of the job they would change if they could, which surfaces ergonomic and psychosocial hazards nobody has words for yet.

Read your own records
First aid entries, incident reports, insurance claims, maintenance requests, and complaints. Repeat entries are a hazard telling you the control failed, and a cluster of maintenance requests in one area is usually a safety hazard wearing a different hat.

Cover the tasks nobody does daily
Cleaning a machine, changing a bulb, taking a delivery in the rain, dealing with a spill, closing alone. Non-routine work has no routine protecting it, which is why it appears out of proportion in serious injuries.

Write each hazard as a scenario
Name the place, the person exposed, the trigger, and the likely injury. A line that reads chemicals is unusable. A line that reads a cleaner decanting concentrate into an unlabeled bottle in a supply closet with no ventilation tells you what to change.

Date it and give every fix an owner
An undated list is indistinguishable from an old list. A name and a target date turn the walkthrough into work that finishes, and they are what makes the next review a comparison rather than a repeat.

For a specific job rather than a whole site, the task-level version of this exercise is a job hazard analysis, which breaks a job into steps and pairs each step with its hazards and controls.

Ranking the Controls So the Fix Holds

Controls are ranked by how little they depend on a person doing the right thing under pressure. NIOSH sets out five levels, from elimination through substitution, engineering controls, and administrative controls, down to personal protective equipment, and describes the upper levels as more effective because they control exposure without significant human interaction (NIOSH, Hierarchy of Controls).

Two things follow. The first is a diagnostic: read the control column of your own hazard list, and if every line says training or protective equipment, you have documented the workplace rather than improved it. At least one serious hazard should have a control that physically changed something.

The second is that protective equipment is a legitimate control in its place, not a failure. It belongs where higher controls are not feasible, while they are being built, where safe work practices alone are insufficient, and in emergencies. The mistake is reaching for it first because it is the fastest thing to buy, which is the same instinct that fills a safety training plan with slides about being careful.

What to Write Down

A hazard you identified and did not record is one that leaves with the person who noticed it. The written artifacts are also what a penalty calculation looks at, since the good faith reduction is assessed on evidence rather than intention.

There is no single federal rule demanding one master hazard document, but several standards require documented assessment in their own areas, and the personal protective equipment rules are the clearest: assess the workplace for hazards that make protective equipment necessary, then verify that assessment in writing. In my experience the assessment gets done in somebody's head and the written certification never does, which is the version that shows up in an OSHA violation file.
