Most small businesses meet a job safety analysis because somebody else asked for one. A general contractor wants a pre-task plan before your crew sets foot on site. An insurer asks what your process is at renewal. A customer sends a vendor packet with a JSA field in it. And the owner, who has been doing the job safely for a decade, now has to write down what has only ever lived in their head.

That is a frustrating place to start, and it is also the wrong reason to do it. The analysis is worth writing for its own sake: it is the cheapest way I know to find out that the reason nobody has been hurt clearing the wrap machine is that everyone has been lucky and careful, in that order.

There are six templates here: a blank JSA worksheet, a printable form for the clipboard, a construction pre-task version, a warehouse and manufacturing JHA, a version for offices, shops, and field service, and one completed example. TL;DR
A job safety analysis (JSA), also called a job hazard analysis (JHA), breaks one job into steps, names the hazard at each step, and assigns controls strongest first. OSHA publishes the technique as guidance, though 29 CFR 1910.132(d) requires a written certification of the PPE hazard assessment. Six free templates below, as XLSX and DOCX. What a Job Safety Analysis Is

A job safety analysis is a worksheet that breaks one job into its steps, lists what could go wrong at each step, and assigns a control to each hazard. OSHA defines the technique as one that focuses on job tasks as a way to identify hazards before they occur, and that looks at the relationship between the worker, the task, the tools, and the work environment (OSHA Publication 3071, Job Hazard Analysis).

The distinction that matters is between a hazard and an incident. A hazard is the potential for harm, usually a condition or an activity that produces an injury if it is left uncontrolled. An incident report documents what already happened. A JSA is the document you write beforehand, so the incident report never gets written.

You do not analyze every job in the company at once. OSHA sets out five categories that earn priority, and for a small business that list is usually enough to fill a quarter of work.

Highest injury or illness rates
The jobs your own records already point at. If one task keeps showing up in first aid reports or claims, it goes to the front of the queue.
Potential for a severe or disabling injury
Analyze these even when nothing has happened yet. A clean history on a high-consequence task is luck, not evidence of control.
One simple human error could be severe
Tasks where a single slip, a moment of reaching, or a wrong switch has a serious outcome, and no second chance to recover.
New jobs, or changed processes
New equipment, a new product line, a new site, or a procedure that changed last month. The old analysis no longer describes the job.
Complex enough to need written instructions
If the job requires written steps to be done correctly, it requires written hazards and controls alongside those steps.
Start With the Job That Scares You
If you are picking a first job to analyze, do not start with the easy one to write up. Start with the task where a single mistake has a serious outcome and nothing has gone wrong yet, because that combination is where a clean record is doing the most work to hide a real hazard. OSHA lists exactly that category as a priority: jobs with the potential to cause severe or disabling injury, even with no history of previous accidents. JSA, JHA, and AHA: The Same Worksheet

Job safety analysis and job hazard analysis are the same document under two names, and OSHA uses both terms itself. Its booklet carries the title Job Hazard Analysis, while the text inside tells employers to consult the OSHA standards for their industry "when conducting your own job safety analysis." Neither term is more official than the other.

The AHA is worth knowing about if you ever bid federal work. Under the US Army Corps of Engineers safety manual (EM 385-1-1), an activity hazard analysis defines the steps in the activity, the anticipated hazards, the site conditions and equipment, and the control measures, and the manual treats contractor JSAs and JHAs as acceptable substitutes when they capture the same data. Your existing form is usually closer to compliant than you expect.

Pick One Name and Keep It
The practical risk in this vocabulary is not confusion about the concept, it is a company with three different form names in circulation: a JSA in the safety folder, a pre-task plan on the truck, and a hazard assessment in the handbook. Auditors, insurers, and new hires all read that as three separate processes, none of which is being followed. Choose the name your customers already use, rename the others, and let the safety policy reference that single term.

One Job Per Analysis
The most common mistake in a first JSA is scope. "Warehouse operations" is not a job, it is a department, and an analysis at that altitude produces hazards so generic that no control can attach to them. "Unload the delivery truck and shelve stock" is a job: it has a beginning, an end, a defined crew, and six or seven steps you can watch. If your step list runs past ten or twelve lines, you have probably captured two jobs, and they will both be served better by being split.

How to Write a Job Safety Analysis

You write a job safety analysis by picking one job, listing its steps in the order they happen, naming the hazard at each step, assigning controls strongest first, and rating what risk is left. OSHA's recommended starting point comes before all of that: involve your employees, review your accident history including near misses, hold a preliminary job review, then list and rank the hazardous jobs.

Every column in the worksheet answers a different question, and mixing them up is what produces an analysis nobody uses.

The hazard column is where most analyses go soft. Writing "slips" or "lifting" records a category, not a hazard. OSHA describes a good hazard scenario as five parts, and the five-part version is what makes the control obvious.

Five parts of a hazard that is worth writing down

EnvironmentWhere it is happening. The stockroom aisle, the dock plate, the third rung of the ladder.

OSHA's own worked scenario reads like this: in the metal shop, while clearing a snag, a worker's hand comes into contact with a rotating pulley, which pulls the hand into the machine and severs the fingers quickly. The word "quickly" is doing real work in that sentence. As OSHA notes, training is not very effective as a control when triggering events happen fast, because people can only react so quickly. That single observation should push you up the control hierarchy every time.

Pick the job and involve the crew
Choose a priority job, talk to the people who perform it, and review your injury and near-miss history for that task first.

List the steps in order
Watch the job being done and record each step with enough detail to describe the action, without splitting it into dozens of lines.

Name the hazard at each step
Write the environment, the exposure, the trigger, the consequence, and the contributing factors rather than a one-word category.

Assign controls strongest first
Work down the hierarchy: eliminate, substitute, engineer, administrative controls, then PPE last rather than first.

Rate what is left and brief it
Record the residual risk, give each fix an owner and a date, then brief the analysis with everyone who does the job and sign it off.
Fix What You Find Immediately
OSHA is explicit that if any hazard you find poses an immediate danger to life or health, you act right then, and that anything easily corrected should be corrected as soon as possible rather than held until the analysis is complete. A JSA that documents an unguarded hazard for three weeks while the paperwork is finished is worse than no JSA at all, because it is now written evidence that you knew. Choosing Controls That Hold

Controls are ranked by how little they depend on a person doing the right thing every single time. NIOSH sets out five levels, from elimination through substitution, engineering controls, and administrative controls, down to PPE, and describes the upper levels as more effective precisely because they control exposure without requiring significant human interaction (NIOSH, Hierarchy of Controls).
