Most small businesses meet a job safety analysis because somebody else asked for one. A general contractor wants a pre-task plan before your crew sets foot on site. An insurer asks what your process is at renewal. A customer sends a vendor packet with a JSA field in it. And the owner, who has been doing the job safely for a decade, now has to write down what has only ever lived in their head.
That is a frustrating place to start, and it is also the wrong reason to do it. The analysis is worth writing for its own sake: it is the cheapest way I know to find out that the reason nobody has been hurt clearing the wrap machine is that everyone has been lucky and careful, in that order.

What a Job Safety Analysis Is
A job safety analysis is a worksheet that breaks one job into its steps, lists what could go wrong at each step, and assigns a control to each hazard. OSHA defines the technique as one that focuses on job tasks as a way to identify hazards before they occur, and that looks at the relationship between the worker, the task, the tools, and the work environment (OSHA Publication 3071, Job Hazard Analysis).
The distinction that matters is between a hazard and an incident. A hazard is the potential for harm, usually a condition or an activity that produces an injury if it is left uncontrolled. An incident report documents what already happened. A JSA is the document you write beforehand, so the incident report never gets written.
You do not analyze every job in the company at once. OSHA sets out five categories that earn priority, and for a small business that list is usually enough to fill a quarter of work.
JSA, JHA, and AHA: The Same Worksheet
Job safety analysis and job hazard analysis are the same document under two names, and OSHA uses both terms itself. Its booklet carries the title Job Hazard Analysis, while the text inside tells employers to consult the OSHA standards for their industry "when conducting your own job safety analysis." Neither term is more official than the other.

The AHA is worth knowing about if you ever bid federal work. Under the US Army Corps of Engineers safety manual (EM 385-1-1), an activity hazard analysis defines the steps in the activity, the anticipated hazards, the site conditions and equipment, and the control measures, and the manual treats contractor JSAs and JHAs as acceptable substitutes when they capture the same data. Your existing form is usually closer to compliant than you expect.

Template 2: Printable Job Safety Analysis Form

How to Write a Job Safety Analysis
You write a job safety analysis by picking one job, listing its steps in the order they happen, naming the hazard at each step, assigning controls strongest first, and rating what risk is left. OSHA's recommended starting point comes before all of that: involve your employees, review your accident history including near misses, hold a preliminary job review, then list and rank the hazardous jobs.
Every column in the worksheet answers a different question, and mixing them up is what produces an analysis nobody uses.

The hazard column is where most analyses go soft. Writing "slips" or "lifting" records a category, not a hazard. OSHA describes a good hazard scenario as five parts, and the five-part version is what makes the control obvious.
OSHA's own worked scenario reads like this: in the metal shop, while clearing a snag, a worker's hand comes into contact with a rotating pulley, which pulls the hand into the machine and severs the fingers quickly. The word "quickly" is doing real work in that sentence. As OSHA notes, training is not very effective as a control when triggering events happen fast, because people can only react so quickly. That single observation should push you up the control hierarchy every time.
Choosing Controls That Hold
Controls are ranked by how little they depend on a person doing the right thing every single time. NIOSH sets out five levels, from elimination through substitution, engineering controls, and administrative controls, down to PPE, and describes the upper levels as more effective precisely because they control exposure without requiring significant human interaction (NIOSH, Hierarchy of Controls).
OSHA adds a point about PPE that small businesses routinely invert. PPE is an appropriate control when engineering controls are not feasible or do not fully eliminate the hazard, while those controls are being developed, when safe work practices are not sufficient on their own, and during emergencies. It is a legitimate control in those cases, and a poor substitute for the ones above it.
A quick diagnostic: read down the control type column of a finished analysis. If every row says PPE or training, the analysis has documented the job rather than improved it. At least one row on a serious hazard should describe something you physically changed, even if it is as small as moving a carton from the floor to a table.
Worked Job Hazard Analysis Examples
A job hazard analysis example is a single job step paired with the hazard it creates and the controls that reduce it. The three below come from a warehouse, a kitchen, and a customer site, and each one shows the level of specificity that separates a form which gets filled in from a form which gets filled in usefully.

Notice the pattern. Each hazard scenario names the place, the person or body part exposed, the trigger, and the consequence, which is exactly the structure OSHA recommends. And in each row, the first control changes the job rather than the person: lock it out, cool it down, use the right ladder. The PPE appears last, after the physical change, not instead of it.

What the Law Actually Requires
Federal OSHA does not require a job safety analysis by name for general industry. It publishes the technique as guidance, and the booklet itself tells employers to consult the OSHA standards for their industry, because compliance with those standards is mandatory while the analysis method is not.
What is required, in specific places, is documented hazard assessment. The clearest example is PPE: an employer must assess the workplace to determine whether hazards are present that make PPE necessary, and must verify that assessment through a written certification.
Construction carries its own version. Under 29 CFR 1926.20(b)(2), an employer's accident prevention program must provide for frequent and regular inspections of the job sites, materials, and equipment by competent persons. A competent person is defined at 29 CFR 1926.32(f) as one who is capable of identifying existing and predictable hazards in the surroundings or working conditions, and who has authorization to take prompt corrective measures to eliminate them. Naming that person on the pre-task form is not a formality.
Two more sources of obligation sit outside the federal standards, and for a small business they often bite first. States that run their own OSHA-approved plans can set standards that differ from the federal ones, as the OSHA booklet itself notes, so confirm your state before assuming the federal position is the whole answer. And commercial pressure is real: a general contractor, a customer, or an insurer can make a JSA a condition of the contract regardless of what any standard says.

Brief It, Sign It, Store It
A finished analysis has no effect until the people doing the job know what it says. The failure mode is not a bad worksheet, it is a good worksheet that lives in a drawer while the job continues exactly as it did before. That is also the version an insurer or an inspector finds least convincing.
